Client Referral & Earnings Partnership Agreement

This agreement outlines a performance-based earning opportunity for individuals who connect Pinewell Home Care with families in need of home care services. Our goal is to build meaningful connections in the community while rewarding genuine, high-quality referrals.

1. Assessment Appointment Bonus

The Outreach Representative will receive $25 for each valid and completed assessment appointment arranged with a potential client.

An appointment will qualify only if:

  • The client has a genuine interest in home care services

  • The appointment is confirmed in advance

  • A Pinewell Home Care representative completes the assessment (in person or phone)

  • The client acknowledges the Outreach Representative as the source of referral

  • The assessment is completed and deemed valid for care evaluation by Pinewell Home Care

Appointments that are cancelled, no-shows, repeatedly rescheduled, or determined to be invalid or non-genuine will not qualify for payment.

Pinewell Home Care reserves the right to determine whether a referral is a legitimate potential client.

2. Client Service Commission (Performance-Based Earnings)

If a referred client begins services, the Outreach Representative will receive additional compensation based on completed and paid service hours within the first 30 days from the start of care.

Commission Structure:

  • 1 – 20 hours: $75

  • 21 – 50 hours: $150

  • 51 – 80 hours: $250

  • 81 – 120 hours: $350

  • 120+ hours: $500

Commission applies only to actual completed and paid hours.
If services are discontinued early or payments are reversed, commission may be adjusted accordingly.

3. Payment Terms

Payments will be issued only after:

  • The client completes the applicable service hours, and

  • Pinewell Home Care has received payment for those services

All eligible payments will be processed within 7–14 business days after the above conditions are met.

4. Referral Submission & Ownership

To qualify, the Outreach Representative must submit referral details at the time of introduction, including:

  • Client full name

  • Phone number

  • Area or address

  • Date of referral

Referrals will be credited to the individual whose information is first recorded by Pinewell Home Care.

Referrals will not qualify if the client is already:

  • In contact with Pinewell Home Care

  • Previously registered

  • Already receiving services

5. Fraud Prevention & Quality Control

Pinewell Home Care reserves the right to reject referrals that are:

  • Fraudulent, duplicate, or misleading

  • Submitted without genuine client interest

  • Created solely to generate compensation

No payments will be issued for invalid referrals.

6. Privacy & Information Handling

The Outreach Representative may only collect basic contact information required to arrange an assessment.

  • Collection of medical or sensitive health information is strictly prohibited

  • All client information must be handled responsibly and shared only with Pinewell Home Care

7. Role & Limitations

The Outreach Representative acts solely as a referral partner and is not an employee, agent, or representative of Pinewell Home Care.

Outreach Representatives must not:

  • Negotiate pricing or services

  • Make promises or commitments on behalf of Pinewell Home Care

  • Misrepresent services or scope of care

Pinewell Home Care is not liable for any unauthorized representations made.

8. Agreement Updates & Termination

Pinewell Home Care reserves the right to modify or terminate this agreement at any time with written notice.
